TOP-ACDNT: Accident

Scope: This procedure covers steps taken when an accident occurs that requires medical attention.

Call 9-1-1 when simple first aid will not help, when trauma or severe injury occurs, or when a person cannot be moved due to their injury!

STEP WHO WHEN INFO TO USE WHAT TO DO RESULTS
1 Anyone who observes accident Accident occurs Personal observation Stabilize injured person, remove from immediate danger (unless head, neck or back injury or other trauma situation). Send word to Foreman or Supervisor. Injured person stabilized and removed from danger if possible; supervisor notified.
2 Any supervisor* When notified of accident Own assessment of injury; List of Medical Facilities According to nature of injury, direct employee to medical facility, or direct pre-designated employee** to drive injured employee to medical facility for treatment. Injured transported to appropriate medical facility for treatment.
3 Supervisor After assessment of injury and if employee is comfortable and able to speak coherently Employee's description of circumstances involving the accident; Employee Statement of Injury; Manager's/Supervisor's Report Complete top section of Authorization for Treatment and Report; have injured sign the Medical Authorization and Drug and Alcohol Screen Authorization and Release sections; instruct injured to submit report to medical facility upon arrival. Help injured complete Employee Statement; or collect and record information to complete Manager's/Supervisor's Report. Authorization completed for injured to receive medical attention; injured authorized release of information and drug and alcohol testing; injured directed to medical facility and instructed to submit authorization forms upon arrival; Employee's statement of circumstances of his injury are recorded on Employee Statement and Manager's/Supervisor's Report.
4 Injured Employee Upon injury and receiving direction to medical facility from supervisor Instructions from supervisor; Authorization for Treatment and Report Go to medical facility for treatment as instructed by supervisor, and submit Authorization for Treatment and Report to medical staff upon arrival. Injured seeks medical treatment and submits proper authorization.
5 Supervisor After accident, before next morning operations meeting Employee Statement of injury, Manager's/ Supervisor's Report, Witness Statement Investigate accident, collect information from any witnesses, complete reports and forward to Human Resources Manager. Manager's/Supervisor's report and Witness Statement completed and forwarded to Human Resources Manager
6 Human Resources Manager Morning following accident in which lost time occurred Accident Report Present Lost Time report and any other findings via e-mail to all managers; issue CAR per TOP-CARRR Managers apprised of lost-time accidents and circumstances; CAR issued
7 Human Resources Manager Periodically, until injured recovers and returns to work Doctor's reports, employee input Review employee's recovery with employee and doctors on an informal and periodic basis; relay status to President or CEO Follow-up maintained during employee recovery; company kept advised of injury, recovery, and therapy status.
8 Injured employee Medical Release Upon recovery Obtain Medical Release from Company doctor; submit to Human Resources Manager. Employee recovered and Medical Release submitted to Human Resources Manager.
9 Human Resources Manager Upon receipt of Medical Release Medical Release Review Medical Release; authorize employee to return to work; file Medical Release in Personnel File. Employee's return to work approved; Medical Release filed in Personnel File.

*Supervisor includes any manager, supervisor, foreman or lead man. If one is not quickly available, any team member can assume the role.

**Pre-designated employee(s) is/are chosen by foremen or leadmen to assist injured employee(s) with transportation to proper medical facility in the event injured employee cannot safely operate a vehicle.
