TOP-BRKDN: Breakdown Repair

SCOPE: This procedure covers the maintenance of equipment other than scheduled preventive maintenance. Primarily this involves breakdown repair, but also covers any unscheduled maintenance that might be undertaken to avoid an imminent breakdown.

2 Any supervisor or manager As determine need to have equipment fixed Own observations or reports from others Report problem to Maintenance Supervisor or Leadman, or to Maintenance Worker. Equipment problem reported to Maintenance
3 Maintenance Supervisor, Leadman or other receiving call As receive request for equipment repair Request for equipment repair; own knowledge of equipment and resources Determine priority of trouble call, who will handle it and when; if problem is not going to be addressed immediately, inform requester of priority and time frame. Work orders to maintenance workers and/or verbal instructions; requester informed
4 Maintenance Worker As assigned Maintenance work order and/or verbal instructions Initiate maintenance work order (if work will take more than a few minutes); perform tasks as assigned; report to appropriate supervisor when complete; report any other work that is needed, any ideas for improving the equipment or its operation, or points that ought to be added to the equipment's PM work order (report to Leadman or Supervisor); mark work done on work order and return it to Supervisor; if furnace temperature controls in need of repair or calibration, inform Maintenance Supervisor. Appropriate supervisor informed when complete; maintenance work order (if needed) completed and returned to Supervisor; any suggestions expressed. Maintenance Supervisor informed of Furnace temperature controls in need of repair or calibration

Maintenance Supervisor As determined a temporary repair to be needed Own observations and evaluations; input from Production Supervisor; plus the definition of "Temporary Repair" as it applies (see note) Once a temporary repair is merited and completed, issue a Work Order in AMMS; have the Work Order show the date the temporary repair is to be replaced by the correct permanent repair; put equipment back in service when repairs completed. Repair completed; equipment back in service
6 Maintenance Manager; VP Manufacturing Work Orders for temporary repairs are past their due date Work Orders for temporary repairs Determine the reason a Work Order for a temporary repair was not completed on time; assign a new due date to the Work Order. Reason given for Work Order not being completed; new due date assigned
7a Maintenance Supervisor When furnace temperature controls are in need of repair or calibration Notification from maintenance worker Issue Work Order; inform Calibrations Clerk via E-Mail so he can note on calibration log; send instrument to approved calibration supplier. Calibrations Clerk informed; calibration log updated; instrument shipped
7b Maintenance Supervisor If furnace temp control fails calibration and cannot be economically repaired Results of calibration inspection Tag instrument with reject label; notify Calibrations Clerk of final disposition of instrument via email. Defective instrument removed from premises
8c Maintenance Supervisor If instrument passes calibration Results of Calibration Notify Calibrations Clerk of calibration date and next calibration date via email; tag instrument with calibration label showing ID, calibration date, next calibration date and vendor name. Calibrated instrument tagged and returned
9 Maintenance Supervisor As Maintenance work orders returned Work orders plus own observations or tests Review any suggestions for improvements; return completed work order. Good suggestions reviewed; completed maintenance work orders back to Warehouse Administrator
10 Warehouse Administrator Weekly Completed maintenance work orders Perform any maintenance job costing requested by Maintenance Supervisor; enter info into AMMS; discard Maintenance work orders. Completed work entered into AMMS; Maintenance Supervisor notified of exceptions

Note: "Temporary repair" is defined herein as "a fix meant to last only long enough to allow for procurement of replacement parts and the scheduling of a full repair, but a fix that nonetheless fully provides for the safety of personnel and equipment."
